The Senior Services Behavioral Health & Wellness program uses a Supports Coordination model to provide supportive services and coordination of care for individuals 55 years of age and better, with severe and persistent mental illness and with concurrent health issues.
Our clinically licensed staff understands mental illness and the many ways it can affect an individual’s ability to achieve a meaningful and rewarding quality of life. Behavioral Health staff utilizes recovery and evidenced-based approaches to address both the psychiatric and the medical needs of the individuals served. Through oversight and monitoring of all necessary services, our experienced staff can improve the communication and coordination of all service providers thus improving treatment outcomes, wellness, and independence. We can assist with the referral processes for housing, money management, health care, health care benefits and entitlements and other needed community resources. Our staff is also able to provide medication management, health education, and recovery-based coaching and counseling. We provide community-based services, meeting with individuals where it is both convenient for them and enables comprehensive care coordination, including residential settings, health provider offices, agency based, and other specialized locales. All of our services are provided in a manner to assure the safety, confidentiality and comfort of the individual receiving our service.
Referrals and funding for the program come through Kalamazoo Community Mental Health and Substance Abuse Services (KCMHSAS); all referrals must be processed through the Access Center.
