Care Navigators are here to help answer difficult questions like this and assist older adults, adults with disabilities, and their caregivers navigate their way to appropriate resources and services. Our Care Navigators are dedicated to help individuals live their lives to the fullest and will work together to help achieve the identified goals.

Trained, caring and professional Navigator staff is available to discuss both short-term and long-term goals and can provide access to hundreds of resources and social supports. With a wealth of knowledge on topics such as nutrition, home repair, home & community based services, and volunteer services, a comprehensive care plan will be completed, and provisions will be made for the coordination of services and supports.

Our Resource Library is an excellent tool which provides a variety of books, tapes, informational sheets, and handouts that help our Navigators develop personalized care plans. Individuals are welcome to peruse the materials and sign out publications to look over in the comfort of their own home. There is no fee for use of the library or the resources.
