Care Navigator and Support Services

The goal of Care Navigator and Support Service is to assist individuals and families with choices and decisions that will help older adults live out their retirement years to the fullest! Our caring professionals will discuss both current and long term needs, and create a comprehensive care plan for your loved one.

The first step is to schedule a needs assessment appointment.  Key components of this assessment include, but are not limited to: Nutrition, Community and Home Based Services, Volunteer Services, and Behavioral and Mental Health Services.  At the completion of the appointment you will be provided with  a care plan to coordinate pertinent services,  along with a list of agencies and programs that can best serve the needs of your loved one.

Our Resource Center Library is an excellent tool which provides a variety of books, tapes, informational sheets, and handouts that help us develop a personalize care plan for your love one.  Caregivers are also invited to pursue the resource materials and may sign out a number of publications that they can look over in the comfort of their home.
